Persons who are engaged in circus or carnival activities must first obtain their license from the City prior to engaging in such activity.

In order to establish, set up, maintain, exhibit, conduct or carry on a circus or carnival in the city, the operator shall first obtain a Temporary Business License prior to commencing operations. Temporary Business Licenses for Circuses or Carnivals are valid for ten (10) days.

Submit a letter from the property owner granting approval for the use of the property for the circus or carnival.

Submit a detailed Site Plan that shows the proposed layout of the circus and/or carnival.  Include tents, rides, trailers, booths restrooms and a detailed electrical layout.

Submit a certificate of insurance listing the City of Wood Dale as a co-insured in accordance with the coverage listed in Sec. 4.403 of the Municipal Code.

Submit a copy of all permits required by other agencies (e.g. Illinois Department of Labor for amusement rides and attractions; DuPage County Health Department for temporary food service, etc.)

As an authorized agent of the entity seeking this license and by signing below and submitting this form, I affirm that the information provided herein is true and accurate to the best of my knowledge.  I acknowledge that I am responsible for and agree to reimburse the City for the cost of inspections conducted by a third party, as required.  Additionally, I acknowledge and agree to comply with the regulations for Circuses and Carnivals as stipulated in Sec. 4.403 of the Municipal Code.
